In a new report, Essential Hospitals Institute presents findings from initiatives at three hospitals to reduce racial disparities in cardiovascular disease (CVD) outcomes.
Produced with support from the Bristol Myers Squibb Foundation, the report, Essential Hospital Innovations to Reduce Disparities in Cardiovascular Care, features projects from Grady Health System, in Atlanta; Harris Health System, in Houston; and Zuckerberg San Francisco General Hospital and Trauma Center, in San Francisco.
The hospitals took a variety of successful approaches to reducing CVD disparities, including targeting social determinants of health and leveraging remote patient monitoring and electronic health records.
