The Increasing Cardiac Rehab Access and Use Among Older Adults at Essential Hospitals Learning Collaborative grant will provide association members a forum to:
- Collaborate and co-develop solutions.
- Learn practical strategies for improving cardiac rehab referral processes and access.
- Share tools for improving care coordination to support integrating cardiac rehab into post-discharge care protocols for older adult patients with chronic conditions.
- Broaden knowledge on reducing preventable readmission rates.
Funding Amount:
Up to 15 member hospitals with up to $100,000 each.

Addressing Health Outcomes for Individuals With Intellectual and Developmental Disabilities (IDD) and/or Rare Diseases (RD)
This PCORI Funding Announcement (PFA) seeks to fund high-impact, patient-centered comparative clinical effectiveness research (CER) studies that focus on people living with intellectual and developmental disabilities (IDD) and/or rare diseases.

Direct costs up to $12 million and up to five years in duration.
