Hennepin Health: A Spotlight on Collaboration

Hennepin Health, a safety-net accountable care organization in Minnesota, has been an exemplary model for delivery system and payment reform for many years. In this webinar, participants were able to learn more about the foundation of Hennepin Health — the collaborative relationship between its founding members, including Hennepin County Medical Center, NorthPoint Health & Wellness Center, and Hennepin County. Representatives from these organizations shared insights and lessons learned regarding working in partnership to move toward value-based care.

This webinar was sponsored by the National Partnership for the Health Care Safety Net — a collaborative project between America’s Essential Hospitals, the National Association of Community Health Centers, and the Department of Health Policy and Management at Milken Institute School of Public Health at the George Washington University. The National Partnership is funded by Kaiser Permanente Community Benefit and focuses on hospital and health center collaboration related to health policy and reforms.
