Network with Essential Hospital Leaders Participating in the IHI Age-Friendly Health Systems Movement

In April 2026, the Essential Hospitals Institute will launch a learning cohort in collaboration with the Institute for Healthcare Improvement (IHI) to support essential hospitals participating in the IHI’s Age-Friendly Health Systems Action Community.

The Age-Friendly Health Systems Action Community is a no-cost opportunity for health professional teams—including essential hospital staff—looking to accelerate the adoption of evidence-based Age-Friendly care in their setting. Action Community participants will test the “4Ms Framework” at their site of care with the goal of being recognized as an Age-Friendly Health System.

With support from the John A. Hartford Foundation, Essential Hospitals Institute will hold supplementary convenings for Action Community participants from essential hospitals. Join us between IHI webinars to delve more deeply into the challenges and opportunities essential hospitals face in delivering Age-Friendly care.

The first IHI Age-Friendly Action Community webinar is March 26. Essential Hospitals Institute’s convenings will begin in mid-April; we will send you a calendar invite upon registration.
