In this webinar, participants learned about the unique partnerships behind an innovative community paramedic program in St. Paul, Minn. To reduce emergency calls and unnecessary hospital readmissions, specially trained paramedics visit homes of discharged patients to provide medication reconciliation, clinical and home assessments, and connections to community resources. Representatives from program partners, Regions Hospital and West Side Community Health Services, shared program details, lessons learned, and insights into the partnership’s success.
