Susan Hawkins, MBA, senior vice president of population health at Henry Ford Health System, shared lessons learned from the health system’s partnership with a large community food bank to pilot a food insecurity program. Participants also heard how the Essential Hospitals Value-Based Care Collaborative can help hospitals integrate their work on social determinants of health with value-based care.
