Research proves that community health workers (CHWs) are effective in achieving the Triple Aim of health care: improving outcomes, increasing client engagement, and reducing costs. As health systems take on more financial risk for their patients, the CHW model can serve as a lower-cost population health management solution for high-risk populations.
Sinai Urban Health Institute (SUHI), in Chicago, has nearly two decades of expertise leveraging CHW chronic disease management models. SUHI has published best practice guidelines on hiring, training, supervising, and integrating CHWs into health care settings. The institute in 2017 launched CROWD (Center for CHW Research, Outcomes and Workforce Development), a CHW training and technical consulting entity supporting organizations that adopt the CHW model.
Participants heard two experts from SUHI share how they developed, implemented, evaluated, and scaled this transformative program to improve access to care in underserved communities.
