3-2-1 IMPACT: Supporting Equitable, Two-Generational Care across Three Disciplines

It has long been recognized that mitigating the effects of trauma during early childhood produces better health outcomes for the physical, emotional, mental health, and intellectual development of children through adulthood. Comprehensive support is needed to most effectively address trauma’s effects, and better outcomes arise the earlier an intervention is made. Promoting economic stability, positive parent-child interactions, and parental support can bolster family resilience and improve the development, growth, and lifelong health trajectories of parents and their children.

Using prevention and equitable, evidence-based care, NYC Health + Hospitals established the 3-2-1 IMPACT clinical model, which represents three disciplines, a two-generation focus, and one goal to improve care for parents and young children.

Participants explored how the health system responds to two-generational needs by optimizing electronic medical records, billing systems, staffing, risk tiering, and supportive workflows across departments and manages workplace culture challenges to better support families throughout the “fourth trimester.”
