The Current Landscape of Population Health in Essential Hospitals and Academic Medical Centers

Researchers from America’s Essential Hospitals and University HealthSystem Consortium (UHC) presented the findings of a content analysis of population health programs in place at their respective member organizations. Additionally, providers from three exemplar programs presented lessons learned in relation to implementation, measures of evaluation, outcomes, and implications for sustainability. All are encouraged to listen and hear about the stories and insights of these experienced population health care providers that aim to improve care for disproportionately vulnerable patient populations.
