Targeting Youth to Improve Population Health

For many social determinants of health, younger populations are a key target for improving health and well being. Teenagers and young adults, specifically, are an important population for effecting change and altering long-term health.

Participants heard from essential hospital Hennepin County Medical Center about its innovative program, Aquí Para Ti (Here for You). This clinic-based development program consults and educates Latino teens and families about topics such as:

  • physical health;
  • relationships with family and friends;
  • body image concerns;
  • mental health;
  • substance use;
  • family planning; and
  • sexual health.

The Minnesota Department of Health recognized Aquí Para Ti as a certified Health Care Home in 2010 and a certified Behavioral Health Home in 2016. Participants heard firsthand about the program and gain insights on challenges and solutions for targeting youth to improve population health.
