Reducing ADEs Through Trigger Tools

Frank Federico, RPh, from the Institute for Healthcare Improvement (IHI), discussed the evolution of the IHI trigger tool methodology and the use of three high alert medication triggers to reduce ADEs. Participants learned how to use this simple method to accurately identify ADEs and the importance of measuring ADE rates over time. Priya Patel, PharmD, a clinical pharmacy specialist at Alameda Health System, shared why they use the high alert medication trigger tool and the medication management process changes they made as a result.
