Rapid Transformation Model Improves Care Delivery, Outcomes

Date: Wednesday, July 22, 2015

Time: 2:00 PM ET

Rapid transformation is a challenge for any organization. Stanford University’s Behnam Tabrizi, author of Rapid Transformation: A 90-day Plan for Fast and Effective Change, has discovered organizations successful at this task share common practices. The practices make up what is referred to as the rapid transformation model.

Santa Clara Valley Health and Hospital System (SCVHHS) is using Tabrizi’s rapid transformation model to manage the increase in service demand after health care reform.

The discussion on rapid transformation covered

  • the principles of the model;
  • the role of system approach, including executive sponsor;
  • key results; and
  • how the model can be applied in any setting or department, from patient admissions to finance to care delivery.

According to Tabrizi, and using SCVHHS as an example, the model can dramatically improve the customer experience, delivery system efficiency, and patient outcomes.

Find out even more details on how SCVHHS integrated its rapid transformation plans with the strategic plan for the medical center and health system as a whole.
