Project ACHIEVE: Results from Phase 1

Patients suffer harm too often as they move between sites of care, and their caregivers experience significant burden. Project ACHIEVE aims to understand and confront these issues through an observational study of transitional care efforts in hospitals nationwide. The webinar highlighted findings from Phase 1 of the project, which included focus groups and key informant interviews with patients, caregivers, and health care providers.
