Eighty percent of what contributes to a person’s health happens outside the traditional clinical health care environment. Without access to support resources, unexpected life emergencies, such as loss of a family member, loss of income, or diagnosis with chronic disease can have lifelong negative effects on individual and family health outcomes.
Participants discovered how association member The MetroHealth System, in Cleveland, takes a practical approach to targeting social determinants of health in the community with its Institute for H.O.P.E. The MetroHealth Buckeye Community Resource Center is rooted in one of Cleveland’s most enervated neighborhoods, challenged by high unemployment, poverty, and infant mortality rates. Participants learned how MetroHealth cultivates an environment of trust among patients, providers, and other stakeholders by placing community-based partners inside the community health center to coordinate care for patients with complex needs.
