One Delivery System’s Perspective on Medicaid Waiver/DSRIP Transformation

NYC Health + Hospitals (Health + Hospitals) is the largest public health care delivery system in the United States and serves 1.1 million patients annually, 80 percent of whom are Medicaid beneficiaries or uninsured. Because of its scale and steadfast mission to provide quality care for all, Health +Hospitals leaders have informed major components of New York State Medicaid redesign over the past five years, and the successes of those redesign efforts led to a reinvestment of savings dollars into New York’s Delivery System Reform Incentive Payment (DSRIP) program. Currently, Health + Hospitals leads a 250-organization collaborative working together under the DSRIP program to transform into a sustainable community-facing delivery system that addresses social determinants of health and achieves improved health outcomes.

It is now DSRIP Year 2 and there are some early insights that may be useful.  This webinar was intended to

  • discuss our approach to building a collaborative of hospitals and community based organizations; and
  • describe achievements, challenges, and early lessons learned.
