Moving to Action for Hospitals and Population Health Update

In January, Essential Hospitals Institute was awarded a Robert Wood Johnson Foundation grant for a nine-month planning initiative to facilitate the adoption and spread of population health at essential hospitals across the United States. “Moving to Action for Hospitals and Population Health” has been designed to set forth a strategy to address barriers and outline solutions for essential hospitals as they create and implement population health programs for their community.

This webinar discussed the findings from the project’s primary research tasks, including an environmental scan, member survey, and key informant interviews. In addition, Institute staff shared their plans for a Population Health Roadmap and multi-year continuation project.
