Interventions to Reduce Acute Care Transfers from SNFs

INTERACT Senior Project Coordinator Laurie Herndon, GNP described the purpose of the INTERACT Quality Improvement Program (Interventions to Reduce Acute Care Transfers) and reviewed key tools in the toolkit, implementation strategies, and lessons learned. Cheryl DiPaolo, RN, MHA, care management at Emerson Hospital, provided the hospital perspective on the INTERACT program. Together they discussed the importance of the INTERACT program and the risks associated with patients who are discharged from a hospital to a Skilled Nursing Facility (SNF).
