Improving Care Transitions: Findings From Project ACHIEVE

Patients experience preventable harm as they move between health care sites, such as hospitals or nursing homes. Poorly managed care transitions can cause emotional distress, side effects from medication changes, and worsening symptoms in patients. Project ACHIEVE, a collaborative research project funded by the Patient Centered Outcomes Research Institute, aims to evaluate and recommend care coordination efforts for hospitals.

Participants heard Project ACHIEVE team members present the results of the project’s first phase, which included more than 200 focus groups and interviews with patients and caregivers, as well as an analysis of survey and readmission data from nearly 400 hospitals and health systems. Participants also heard what transitional care services matter most to patients and caregivers, and explored the relationships between transitional care strategies employed by hospitals and readmission rates.
