Helping Healthy Youth Transition out of Foster Care

Youth transitioning out of foster care face significant social, medical, and mental health care needs. While all adolescents face challenges as they move into adulthood, youth in the child welfare system often lack emotional and economic support from their families, and many do not have health insurance.

Participants heard how staff at Memorial Healthcare System, in Hollywood, Fla., responded to this need with the Healthy Youth Transitions (HYT) program, which links youth and young adults to a medical home and targets social determinants of health, including housing, employment, education, money management, and life skills. HYT seeks to provide the necessary, out-of-the-box, community-based services that help propel youth into successful, productive, independent, and responsible adulthood.
