COVID-19 Transitions Clinic: Reducing Transmission and Hospitalization

With COVID-19 hospitalization rates rapidly rising, it was critical that association member Upstate University Hospital, in Syracuse, N.Y., maintain capacity to manage the surge.
To reduce the need for COVID-related emergency department (ED) visits, mitigate preventable hospitalizations, prevent readmissions for hospitalized patients, and minimize COVID exposure in theED and community, leaders launched a COVID-19 Transitions Clinic.
As the virus’ Delta variant poses continued challenges for hospitals, join us to hear staff discuss the clinic and its contribution to reducing hospital stays, ED visits, and community spread.
