Community Partnerships to Target Food Insecurity

Participants learned how essential hospital Henry Ford Health System, in Detroit, forged community partnerships to target social determinants of health. Susan Hawkins, MBA, senior vice president of population health at Henry Ford Health System, shared lessons learned from the health system’s partnership with a large community food bank to pilot a food insecurity program. Participants also heard how the Essential Hospitals Value-Based Care Collaborative can help hospitals integrate their work on social determinants of health with value-based care.
