Care Plans as Tool to Engage Patients and Improve Team-Based Care

Learn about Cambridge Health Alliance’s (CHA) patient-centered medical home (PCMH) model and how it is leading to improved health outcomes. Substantial evidence shows that patients who are more active and engaged in their health have better outcomes and experiences with care. The Agency for Health Research and Quality considers patient engagement a cornerstone of the PCMH model for primary care. Over the past three years, CHA has rolled out a comprehensive care planning program for patients with uncontrolled chronic diseases and complex medical and social needs.

During this webinar, we discussed

  • evidence of effective patient activation and engagement leading to improved health outcomes, experiences with care, and decreased cost;
  • the experience of an integrated health care delivery system to implement a comprehensive care planning program for patients; and
  • the impact of care planning on patient engagement, patient experience, and improved health.
