Health literacy is critical for delivering high-quality, patient-centered care to diverse populations, and it has become a national priority. Adults with limited health literacy experience more serious medication errors, higher rates of emergency department visits and hospitalizations, worse preventive care and health outcomes for their children, and increased mortality. During this presentation, two members of America’s Essential Hospitals discussed the implications of low health literacy and the tools needed to improve patient education and ensure appropriate coordination across health care settings.
