Building Health and Well-Being Through Partnerships

Date: Wednesday, June 1, 2016

Time: 2:30 PM ET

Join Dialogue4Health for their June 1 webinar to learn how partnerships can build community health and well-being. This webinar, cosponsored by America’s Essential Hospitals, is one of several in Dialogue4Health’s Community Prevention and Multi-Sector Stakeholder Web Forum Series.

By leveraging the assets and commitments of multiple sectors, community development initiatives play a critical role in advancing community health, safety and equity. Across sectors, a shared opportunity is emerging to increase access to the elements that underpin health, such as stable, affordable, healthy, and safe housing, transit and active transportation, healthy food access, literacy and learning, job opportunities and health care for individuals and families. Health systems and health departments are well positioned to form partnerships with those in housing organizations, city government, and other sectors to address the key social determinants of health and collectively improve the health and well-being of their communities.

Please join Dialogue4Health for their Web Forum where they will discuss lessons learned from communities taking action to leverage community development efforts through diverse partnerships to improve population health and equity. The conversation will highlight the role of partnerships in financing, evaluating, and sustaining population health improvements. Presenters will also provide recommendations for success and take participant questions.
