How to Build an Accountable Community for Health

Learn how health care, public health, and community organizations formed a structured, community-centric alliance—the Accountable Community for Health—to target social determinants, reduce disparities, and improve health outcomes in Harris County, Texas, home to the nation’s largest population of uninsured people.

This session will share lessons from two years of development, operation, and continued evaluation of an alliance anchored by essential hospital Harris Health System, MD Anderson Cancer Center, Memorial Hermann Health System, and the UTHealth School of Public Health. Participants will learn practical ways to identify a geographic area of focus and connect to shared institutional interests; align with community priorities and trusted assets; target priority needs and outcomes (e.g., food insecurity, HbA1c control); and integrate into health care workflows in a cost-effective manner.
