Bringing It Home: Home Visits for Population Health

Environmental health triggers in homes, schools, and workplaces can cause poor health outcomes and avoidable hospital admissions. Participants heard how UMass Memorial Health Care’s Gage Award–winning program uses home visits to tackle environmental triggers and reduce asthma-related hospitalizations and school absenteeism. Program staff and clinicians shared insights learned from building a business case for the program, leveraging community health workers, and forging community partnerships.
