Universal Social Needs Screening and Population Health

Social needs screening, a key component of population health improvement, can help identify patient needs, inform connections with community partners, and promote upstream interventions. In this webinar, participants heard how Boston Medical Center launched WE CARE, a universal social needs screening program, and learned best practices for designing a patient-centered, equitable screening tool. Attendees gained an understanding of how to connect population health, clinical integration, and quality improvement to effectively screen patients and, in response, develop social needs interventions
