The Robert Wood Johnson Foundation (RWJF) released its annual County Health Rankings report in collaboration with the University of Wisconsin Population Health Institute.
The County Health Rankings offer information on health behaviors, clinical care, social and economic factors, and physical environment. The related Action Center and individual state summary reports provide guidance on how to use the data to take action. The interactive county road map provides county-level data on various health outcomes, health factors, policies, and programs.
The Essential Hospitals Institute Road Map to Community-Integrated Health Care, developed with funding from RWJF, is another resource to help health systems advance population health through activities and a community needs assessment.
As essential hospitals continue work to improve population health, these tools can be helpful for identifying the needs of the communities they serve, allocating community benefit resources, and implementing community-integrated health care.
