CVD Risk Payment Model Applications Due Sept. 10

Providers interested in participating in a new Medicare payment model to prevent cardiovascular disease (CVD) and reduce related mortality and costs have until Sept. 10 to submit an application. The Million Hearts: CVD Risk Reduction Model will incentivize providers to calculate CVD risk for eligible Medicare beneficiaries and target those with the highest risk for patient-centered care. Patients and providers will work together to develop individualized plans that include shared decision making and self-monitoring  to reduce patients’ chances of heart attack and stroke. Provider incentives will be based solely on the provider’s ability to reduce predicted risk, rather than using patient diagnostic benchmarks. The Centers for Medicare & Medicaid Services will enroll up to 720 practices in this new model, which is unlikely to include an additional reporting burden for providers.
