Hospitals, health care practices, and other community-based organizations can apply to participate in a newly revised funding opportunity in the Accountable Health Communities (AHC) Model.
The AHC Model aims to close a gap in the health care delivery system between clinical care and community services.
The Centers for Medicare & Medicaid Services (CMS) will award each participating cooperative agreement up to $1.17 million over five years through Track 1 of the AHC Model. Participants will deploy a screening assessment for health-related social needs to all Medicare and Medicaid beneficiaries seeking care at participating clinical sites. The assessment will test whether increased awareness of available services through information dissemination and referral effects the use and total cost of health care services provided.
CMS will host a webinar on Sept. 14 at 2 pm ET to discuss application changes to Track 1 of the AHC Model. Registration is required.
