Boston Medical Center has released an update to Project RED (Re-Engineered Discharge), a toolkit focused on discharge safety and reducing hospital readmissions. Included in the update is a new chapter, developed with the United Hospital Fund, that provides guidance to hospitals on how to best integrate family caregivers into the discharge plan and at-home health management through a five-step process.
Project RED is overseen by a research group at Boston Medical Center that develops and tests strategies to improve the hospital discharge process and promote patient safety. Since RED’s first release in 2007, more than 500 hospitals worldwide have downloaded the resource.
