The Centers for Medicare & Medicaid Services (CMS) is accepting applications and letters of intent for the Comprehensive End-Stage Renal Disease (ESRD) Care Model. The Comprehensive ESRD Care Model will test the effectiveness of new payment and service delivery models intended to improve quality of care for Medicare beneficiaries with ESRD. Through this model, groups of health care providers and suppliers, called ESRD seamless care organizations (ESCOs), will be responsible for all care offered to a group of Medicare beneficiaries with ESRD, not just care they receive related to their ESRD.
Applications and letters of intent are due June 23 for ESCOs that include a large dialysis organization – those that have more than 200 dialysis facilities – and Sept. 15 for ESCOs that do not include a large dialysis organization. Follow the steps below to apply:
- Create a username and password.
- Submit a letter of intent via the electronic portal.
- Submit an application via the application portal.
