The Centers for Medicare & Medicaid Services (CMS) is accepting applications for the Kidney Care Choices (KCC) and Primary Care First (PCF) Medicare models.
Kidney Care Choices Model
The voluntary KCC model aims to reduce Medicare expenditures and improve care for beneficiaries with end-stage renal disease and chronic kidney disease. The model contains four options:
- CMS Kidney Care First ;
- Comprehensive Kidney Care Contracting (CKCC) Graduated ;
- CKCC Professional ; and
- CKCC Global .
Applications are due to CMS Jan. 22, 2020.
Primary Care First Model
The PCF initiative offers two voluntary five-year models: general and high-need, seriously ill population. Assuming risk based on cost and quality performance, participants will receive a population-based payment and a flat primary care per-visit fee.
Practices must apply by Jan. 22, 2020. CMS in the coming weeks will release a PCF Statement of Interest form from prospective payer partners. A formal Solicitation of Payer Partnership will run Dec. 9, 2019, to March 13, 2020.
CMS will hold an educational webinar about the application process on Oct. 30 from 3–4 pm ET and a webinar focused on the seriously ill population track on Oct. 31 from noon–1 pm ET.
