Survey for Essential Hospitals’ Community Development Partners

Does your hospital partner with, engage with, or know any community development corporations in your community?

Build Healthy Places Network (BHPN) invites community development corporations to complete a 15-minute survey to inform its Community Innovations project. Supported by the Kresge Foundation, which also supports some Essential Hospitals Institute work, Community Innovations will enable capacity building, resource development, and peer connections between community development corporations and health care partners to advance equity, reduce poverty, and improve health outcomes.

As the first step in this project, BHPN hopes to learn from local community development corporations about how they are collaborating or planning to collaborate with health care partners to improve health and well-being in historically marginalized communities.

Findings from this survey will inform new tools, resources, and outreach methods tailored to local organizations. The survey results will be shared in the aggregate in a summary document; individual responses will not be shared publicly. Responses are due Friday, Feb. 7.
