The Robert Wood Johnson Foundation (RWJF) seeks stories of partnerships between health systems, hospitals, patients, and advocates as part of its new Power of Partnership (POP!) initiative.
Through POP!, RWJF wants to learn how successful partnerships have overcome obstacles and share both the successes and the challenges that accompany systemic change. Health system leaders are encouraged to share how a consumer or group of consumers has changed the way the organization practices. Patients and patient advocates are encouraged to share how a clinic, hospital, or other organization incorporated their suggestions into the organization’s operations.
Share your story by answering these five questions in 250 words or fewer:
- Identify your organization and location.
- What did you do?
- What changed?
- Who was involved?
- Provide your name, email address, and phone number. Your contact information will not be shared publicly.
