Remarkable Population Health Programs Across the Nation

Essential hospitals across the nation are employing population health programs aimed at improving the health of people in their communities.

These programs target the underlying causes of poor health outcomes among the most vulnerable patients by promoting healthful lifestyles, improving access to proper nutrition, and increasing access to preventive health care services. Often these hospitals partner with community-based organizations, creating strong networks.
