Institute Launches Essential Communities in Action Video Series

Essential Hospitals Institute has released a series of five videos highlighting population health programs at essential hospitals.

The video series, Essential Communities in Action, can be found on essentialcommunities.org, an online hub for population health-related resources, member best practices, and learning opportunities.

Each video focuses on a unique program dedicated to a specific social determinant of health. Viewers can take an in-depth look at how the hospitals are working with partners to improve health and wellbeing in their communities. For more examples of population health efforts, visit the community program map, which features more than 40 programs around the country.
