Funding Opportunity for Social Determinants of Health Work

The Association of State and Territorial Health Officials and National Association of County and City Health Officials (NACCHO) will award $50,000 grants to a maximum of 50 communities for work to improve health equity by targeting social determinants of health.

The Improving Social Determinants of Health — Getting Further Faster project will support multisector coalitions that target social determinants of health outcomes related to chronic conditions, such as food insecurity, social connectedness, and clinical-community linkages.

Applications are due Dec. 8 at 5 pm ET. More information, including frequently asked questions and an informational webinar recording is available.
