How does Synthroid work? What are its effects?

Levothyroxine is a thyroid hormone supplement used to treat people whose thyroid gland does not produce enough hormones. Levothyroxine reduces the symptoms of hypothyroidism such as weight gain, sensitivity to cold, lack of energy and dry skin. It may take several weeks before the drug begins to produce a noticeable effect on your condition.

Synthroid is also used to reduce the volume of the thyroid gland when it is increased, a condition known as goiter.

Synthroid is available under various brand names or in different presentations. How should Synthroid be used?

The dose of levothyroxine is established according to the level of hormone necessary for the regulation of blood concentration. Laboratory blood tests will help determine the total amount of the drug that a person should take. The initial dose varies according to your general health and depending on the severity and duration of the symptoms caused by low thyroid hormone levels.

You should take Synthroid 1 time a day, and at the same time, to achieve a constant effect. Ideally, it should be taken 30 minutes to 1 hour before lunch; however, it is especially important to take it the same way every day. The dose of levothyroxine should be taken at least 4 hours before or after medications that modify its absorption. The consumption of cottonseed flour, dietary fiber, soy bean flour (for infants) or nuts, can decrease the absorption of levothyroxine.

For infants and children who cannot swallow levothyroxine tablets, Synthroid can be crushed and added to a small volume (5 to 10 mL) of water, breast milk or soy-free formula. This mixture can be administered using a spoon or a dropper. The tablets can also be crushed and sprinkled on a little food such as applesauce. Do not store crushed tablet mixtures.

It could be that your thyroid hormone dose is too high if you notice signs such as chest pain, increased heart rate, palpitations, excessive sweating, heat intolerance and a nervous state. If you think that your thyroid hormone dose is probably too high, consult your doctor as soon as possible.

Several factors can come into play when determining the dose a person needs: their weight, their state of health and the intake of other medications. If your doctor has recommended a dose other than those indicated here, do not change the way the medicine is used without consulting him first.

  • if you are pregnant and taking medication to treat hyperthyroidism.

What are the possible side effects of this medication?

Many drugs can cause side effects. A side effect is an undesirable response to a drug when it is taken in normal doses. It can be mild or severe, temporary or permanent.

The side effects listed below are not experienced by all people who take Synthroid. If you are worried about side effects, discuss the risks and benefits of Synthroid with your doctor.

At least 1% of people taking Synthroid have reported the following side effects. Many of these side effects can be managed and a few may go away on their own over time.

Consult your doctor if you experience these side effects and if they are serious or bothersome. Your pharmacist may be able to give you advice on what to do if these side effects appear :

  • from the hustle and bustle;
  • hair loss (temporary) - especially affecting children during their 1st month of treatment);
  • abdominal cramps;
  • cramps in the legs;
  • a stomach upset;
  • a diarrhea;
  • muscle weakness;
  • a fever;
  • fluctuations in appetite;
  • fluctuations in body weight;
  • headaches;
  • changes in the menstrual cycle;
  • nervousness or irritability;
  • a sensitivity to heat;
  • increased sweating;
  • a tremor (a shaking);
  • sleep disorders.

Most of the side effects listed below do not occur very often, but they could however cause serious problems if you do not get medical attention.

Check with your doctor as soon as possible if any of the following side effects occur :

  • from anxiety;
  • stiffness or joint pain or a limp in a child;
  • mood swings;
  • vomiting.

Stop taking the medicine and seek medical attention immediately if there is a response such as :

Are there any other precautions for use or warnings?

Before using any medication, be sure to inform your doctor about any medical conditions or allergies you may have, the medications you are using and any other important facts about your health. Women should mention whether they are pregnant or breastfeeding. These factors could have an influence on how you should use Synthroid.

Seizures: In some rare situations, people who have taken levothyroxine have suffered seizures when they started taking the drug. If you have a history of seizure disorder, discuss with your doctor how this medication may affect your medical condition, how your medical condition may affect the dosing and effectiveness of this medication, and whether any special monitoring is needed.

Bone density: levothyroxine can cause thinning of the bones. Signs of taking too much or too little medication: Contact your doctor as soon as possible if you notice signs related to taking too much medication (such as chest pain, confusion, fast or irregular heartbeat, mood swings, muscle weakness, psychosis, extreme restlessness, yellowing of the whites of the eyes or skin, or shortness of breath) or signs related to taking too little medication (such as clumsiness, cooling, constipation, dry skin and puffiness, a lack of energy, a vague muscle ache, drowsiness, fatigue, weakness or weight gain).

Breastfeeding: only a small amount of thyroid hormone passes into breast milk. Taken in appropriate quantities, Synthroid did not harm the infants of the women who used it.

Can other agents interact with Synthroid?

There could be an interaction between levothyroxine and any of the following agents :

  • nicotinic acid;
  • amiodarone;
  • non-steroidal anti-inflammatory drugs or NSAIDs (e.g. diclofenac, ibuprofen, ketorolac, naproxen);
  • antacids (e.g. aluminum hydroxide, calcium carbonate, magnesium hydroxide);
  • anticonvulsants (e.g. carbamazepine, phenytoin, phenobarbital);
  • tricyclic antidepressants (e.g. amitriptyline, imipramine);
  • antidiabetic drugs (e.g. glyburide, insulin, metformin, rosiglitazone);
  • apalutamide;
  • beta-blockers (e.g. metoprolol, propranolol);
  • calcium carbonate;
  • some medicines whose name ends in "statin" (e.g. lovastatin, simvastatin);
  • cholestyramine;
  • ciprofloxacin;
  • the colesévélam;
  • the colestipol;
  • corticosteroids (e.g. dexamethasone, hydrocortisone, prednisone);
  • diazepam;
  • digoxin;
  • thiazide diuretics (e.g. hydrochlorothiazide);
  • furosemide;
  • heparin;
  • proton pump inhibitors or PPIs (e.g. esomeprazole, lansoprazole, omeprazole);
  • selective serotonin reuptake inhibitors (SSRIs; e.g. citalopram, fluoxetine, paroxetine, sertraline);
  • iodide;
  • lithium;
  • maprotilin;
  • methimazole;
  • metoclopramide;
  • multivitamins and minerals containing iron or folic acid;
  • the octreotide;
  • estrogens (e.g. conjugated estrogens, estradiol, ethinyl estradiol);
  • orlistat;
  • the "appetite suppressant" pills;
  • birth control pills containing estrogen;
  • polystyrene calcium sulfonate;
  • propylthiouracil;
  • raloxifene;
  • rifampin;
  • the semaglutide;

  • simethicone;
  • somatropin;
  • sucralfate;
  • sodium polystyrene sulfonate;
  • iron supplements (e.g. iron fumarate, iron gluconate, iron sulfate);
  • magnesium supplements (e.g. magnesium hydroxide, magnesium oxide);
  • tamoxifen;
  • testosterone;
  • theophyllines (e.g. aminophylline, oxtriphylline, theophylline);
  • warfarin.

If you are taking any of these medicines, consult your doctor or a pharmacist. In your case, your doctor may ask you to :

  • stop taking any of the medicines,
  • replace one of the drugs with another,
  • change the way you take one or both of the medicines,
  • do not change anything at all.
