The Current Landscape of Population Health in Essential Hospitals and Academic Medical Centers

Researchers from America’s Essential Hospitals and University HealthSystem Consortium (UHC) presented the findings of a content analysis of population health programs in place at their respective member organizations. Additionally, providers from three exemplar programs presented lessons learned in relation to implementation, measures of evaluation, outcomes, and implications for sustainability.   All are encouraged to listen and hear about the stories and insights of these experienced population health care providers that aim to improve care for disproportionately vulnerable patient populations. Participants should consider the following questions, which guided the discussion:

  1. How do you define population health?
  2. What are some challenges to practicing population health?
  3. What can researchers, policy makers, clinicians, and/or communities do to promote the field of population health?
  4. What unique roles do essential hospitals and academic medical centers have in providing population health programs and services? How does this role differ from hospitals that do not have a commitment to serve the underserved?
  5. What would you propose as measures or metrics for measuring the success of population health programs?
