The Center for Medicare & Medicaid Innovation (CMMI) has announced 516 awardees selected to participate in its Million Hearts Cardiovascular Disease Risk Reduction Model for Medicare fee-for-service beneficiaries. Several members of America’s Essential Hospitals are among the participants.
Providers participating in the voluntary risk reduction model will work with patients to develop individualized plans that include shared decision-making and self-monitoring to reduce patients’ chances of heart attack and stroke. Provider incentives will be based solely on the provider’s ability to reduce predicted risk, rather than using patient diagnostic benchmarks. Potential treatment methods include
- smoking cessation interventions;
- blood pressure management; and
- use of cholesterol-lowering drugs
Heart disease is the leading cause of death in the United States, resulting in more than $300 billion in health care costs annually, according to the Centers for Disease Control and Prevention.
