During this session, EHEN Improvement Coach Ed Mendez facilitated a roundtable discussion around what HAC reduction teams are learning from their tests of change and addressed key drivers.
The select drivers by condition for this session were:
- HAPU: Patient / Staff / Family Awareness of Risk (i.e., develop documentation tools to prompt daily skin inspections)
- Falls: Patient / Staff / Family Awareness of Fall Risk (i.e., visual/audible cues)
Los Angeles County + University of Southern California Healthcare Network’s pressure ulcer team also shared how they reduced pressure ulcers by more than 70 percent between 2010 and 2012.
