Neighborhood Partnerships to Improve the Health of Communities

Poor access to care often results in increased chronic conditions, inappropriate use of emergency services, and other unmet health needs. Greenville Health System (GHS) created the Patient-Centered Medical Neighborhood (PCMN) model, which seeks to improve access to care and provide person-centered care to uninsured residents of at-risk neighborhoods who have limited access to care.

Participants heard how the PCMN model contributed to a 15.4 percent drop in outpatient emergency room visits and improved access to care for patients in need. Hospital leaders shared their experiences with this multidisciplinary program, successes, and lessons learned.
