PRAPARE: Using Health IT for Population Health

In this webinar, participants learned about the Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences (PRAPARE). This innovative tool helps essential providers track, measure, evaluate, and respond to the social determinants of health their patients face. Additionally, PRAPARE aligns with important areas of health information technology (HIT) within hospitals and health systems, including ICD-10 coding, electronic health records, and meaningful use regulations. Attendees heard about the benefits of using PRAPARE as well as its critical elements — the core measures assessment tool, the Implementation and Action Toolkit, and electronic health record templates.
