Behavioral health providers and primary care providers treat patients for issues related to mental health. In many systems, these departments are siloed, leading to mental health problems being underdiagnosed and undertreated in busy primary care practices.
In response, Santa Clara Valley Health and Hospital System, which serves 64,000 primary care patients, piloted a program designed to integrate behavioral health and primary care using the IMPACT model as a framework. Results from the initial phase showed increased identification and treatment of patients’ behavioral health needs across diverse patient groups.
During this webinar, participants learned how to use a team-based approach and adapt and use the IMPACT model to successfully integrate care in an ambulatory care setting. Speakers also shared several case studies that demonstrate how patients with straightforward and complex issues benefited from this collaboration.
